**To Fill out these Adobe Forms, you must have Adobe Reader(below). When you open the form in Reader, you will be able to directly type into the form and print it out; however, you will not be able to save changes. You can also Save these PDF's to your hard-drive and fill them out later. To type into the form, simply click on the top of the form, and your mouse will go to the first field that you will be able to fill in. When complete, go to" File", "Print", then mail the form to the address provided on the form.

All agencies are urged to apply for additional funding under this program. The amount of your state Body Armor Replacement grant will not be affected by funding awarded under the federal BVD program. Information on the federal BVP program is listed at the above web-site. Please be advised that the Division of Criminal Justice has no administrative responsibilities with respect to the Federal Bulletproof Vest Partnership Program.
